About Us

Lakeside Chronic Health Institute (LCHI) is a nonprofit innovation hub committed to reimagining how chronic disease is prevented, treated, and managed in historically underserved communities.
Rooted in the belief that health equity starts with community power, we bring together healthcare providers, research partners, and local organizations to close gaps in access and outcomes. Our work is built on four pillars:
